What is Insulin?
Insulin is a hormone produced by the pancreas. Its main role is to help glucose (sugar) move from the bloodstream into the body's cells, where it can be used for energy.
When the body becomes less responsive to insulin, the pancreas may produce more insulin to maintain normal blood glucose levels. This condition is commonly referred to as insulin resistance.
What is a Fasting Serum Insulin Test?
A fasting serum insulin test measures the level of insulin in your blood after an overnight fast.
The test may provide information about how much insulin your pancreas is producing when you have not eaten for several hours.

After eating, blood glucose generally rises. The pancreas responds by releasing insulin to help the body handle this glucose.
A PP insulin measurement can therefore provide additional information about the body's insulin response after food.
The exact timing and method of the test should follow the laboratory's instructions or the treating doctor's prescription.

What Does a High Insulin Level Mean?
A high insulin level can occur for several reasons. One common possibility is insulin resistance, where the body requires more insulin to maintain blood glucose levels.
However, a high insulin result does not automatically mean that a person has diabetes.

Body weight and waist circumference

What Does a Low Insulin Level Mean?
A low insulin level may occur when the pancreas is producing less insulin. The significance depends greatly on the corresponding blood glucose level and the clinical situation.
For example, low insulin with high blood glucose can have a different significance from low insulin with normal or low glucose.
Therefore, insulin results should not be interpreted without considering glucose levels.
Can Insulin Resistance Exist With Normal Blood Sugar?
Yes. In the early stages of insulin resistance, the pancreas may produce additional insulin to keep blood glucose within the normal range.
As a result, blood glucose can remain normal while insulin levels are elevated.
This is one reason why insulin and glucose measurements may sometimes be evaluated together in selected patients.
How Should You Prepare for a Fasting Insulin Test?

Generally:
Fasting is usually required for a fasting insulin test.
Water is generally permitted during fasting.
Avoid food, calorie-containing beverages and alcohol during the fasting period.
Avoid strenuous exercise before the sample unless your doctor advises otherwise.
Inform your doctor/laboratory about medicines and supplements you are taking.
Do not stop prescribed medicines unless your doctor specifically tells you to.
For PP insulin, follow the laboratory's instructions regarding the meal and exact timing of blood collection.
Why Are Both Fasting and PP Insulin Sometimes Ordered?
The fasting test provides information about insulin levels in the fasting state, while the PP measurement provides information about the body's insulin response after food.
When combined with glucose measurements, these results can give the treating doctor a broader picture of glucose-insulin metabolism.
However, there is no single insulin value that by itself diagnoses insulin resistance or diabetes.
Important: Don't Interpret Insulin Reports in Isolation
Insulin levels can vary according to the testing method, timing, fasting duration, recent food intake, medications and individual metabolic factors.
Therefore, a laboratory report should be interpreted using the reference range provided by that laboratory and in the context of the patient's clinical condition.
